Applicant is owner of house but lot is owned by another private individual:

1)      Fire Safety Installation Clearance (City Fire Dept.)

2)      Certificate of Final Electrical Inspection (City Engr’s Office)

3)      Certificate of Occupancy (City Engr’s Office)

4)      Undertaking of Lot Owner to be jointly and severally liable with applicant

5)      Undertaking of Private Lot Occupant to allow disconnection of service

 

Applicant is lessee of premises:

1)      Fire Safety Installation Clearance (City Fire Dept.)

2)      Certificate of Final Electrical Inspection (City Engr’s Office)

3)      Certificate of Occupancy (City Engr’s Office)

4)      Lease Contract

5)      Undertaking of Lot/Building owner to be jointly & severally liable

 

Applicant is a buyer of a mass-housing unit:

1)      Fire Safety Installation Clearance (City Fire Dept.)

2)      Certificate of Final Electrical Inspection (City Engr’s Office)

3)      Certificate of Occupancy (City Engr’s Office)

4)      Contract to Sell between developer and applicant

5)      Certification from developer that applicant is current in paying amortizations

 

Applicant is a successor:

1)      Fire Safety Installation Clearance (City Fire Dept.)

2)      Certificate of Final Electrical Inspection (City Engr’s Office)

3)      Certificate of Occupancy (City Engr’s Office)

4)      Transfer Documents such as:

  1. Deed of AbsoluteSale
  2. Deed of Donation
  3. Deed of Adjudication
  4. Partition among surviving heirs
  5. Waiver of Rights, etc.

 

Application is being processed by an authorized representative:

1)      Fire Safety Installation Clearance (City Fire Dept.)

2)      Certificate of Final Electrical Inspection (City Engr’s Office)

3)      Certificate of Occupancy (City Engr’s Office)

4)      Special Power of Attorney or Notarized Authorization Letter to process application and sign the Contract for        Electric Current Service
